i need questions for employee motivation survey questionnaire.can anybody plz help me in this?soon as possible

i am doing a survey in a hospital and need to make a questionnaire to track down the responses by the staff and employees.my project is titled “Employee motivation” and i need questions on that.the questions must be based upon employee motivation and not employee satisfaction.for ex the question:what motivates u to work?(A)Quality of work (B)salary enhancements and promotions (C)community service.i need to frame a total of 15 multiple choice questions.and i need to do it soon.if anyone could help me with it i’d be very grateful.thanks in advance!
in response to ol’run: no this is not my job. i’m a college student, this is my final year project and i’m struck up in a tight spot cuz my incharge is ill and she cannot guide me, i’m left with no option but to do it myself, cuz my project deadline is approaching fast.PS:i dont earn so i cannot pay u, so, reply at ur own risk as u cannot be paid

do u think your ogr’n can do more than what they are doing now to motivate you?

Employee Engagement is made up of three things: motivation, satisfaction, and effectiveness. You should be careful not to limit your questions to motivation.
